Lower Back Pain Treatment and Surgery in India - Best Spine Hospital India

Pain or discomfort in the lower portion of our back and the spinal column is a common phenomenon and can be largely attributed to our changing lifestyles.

Wrong posture or inadequate exercise is most often the cause of the pain in the lower spinal column. The treatment of such a pain can vary from a simple over the counter medicine to surgery depending on the severity and the cause.

The pain is generally felt in the lumbar and the sacral region and can also affect other areas. A sprain in the muscles or the ligaments constituting the lower portion of the spine can be the most common cause of backache. Some of the other problems that involve pain in the lower back are disc degeneration due to arthritis or ageing, spondylitis, infections and tumors.

In most of the cases the pain is localized in the lower back and vanishes after some time. However, it may even extend to the buttock or the leg on the affected side. In cases wherein the pain is persistent and increases with time or results in weakness in the foot or loss of bladder control- proper medical check up becomes essential.

In some cases stiffness or persistent pain around the spine can disturb your sleep resulting in further problems like tiredness. Such problems can be rectified by the used of pillows designed to maintain our body posture and thus ensure proper sleep. The use of the wrong type of mattresses can also affect the body posture and result in back pain.

Lower back pain is one of the commonest problems afflicting people around the world at some point of time in their life.

In the initial stages of pain, stretching exercises work effectively as Lower Back Pain Treatment.

The lower back supports the upper body and allows the body to turn, twist, bend, lift and walk. It plays a very important role in mobility. The lower back takes a lot punishment, as we tend to overuse or misuse it.

There are many options available for Lower Back Pain Treatment, depending upon the problem.

Surgery is usually the last resort in the treatment of back pain. It is usually only recommended if all other treatment options have been tried or in an emergency situation. The main procedures used in back pain surgery are discectomies, spinal fusions, laminectomies, removal of tumors and Vertebroplasty.

There are different types of surgical procedures that are used in treating various conditions causing back pain. Nerve decompression, fusion of body segments and deformity correction surgeries are examples. The first type of surgery is primarily performed in older patients who suffer from conditions causing nerve irritation or nerve damage. Fusion of bony segments is also referred to as a spinal fusion and it is a procedure used to fuse together two or more bony fragments with the help of metalwork. The latter type of surgery is normally performed to correct congenital deformities or those that were caused by a traumatic fracture. In some cases, correction of deformities involves removing bony fragments or providing stability provision for the spine. A time-tested procedure to repair common intervertebral disc lesions which offers rapid recovery (just a few days) involves the simple removal of the fibrous nucleus of the affected intervertebral disc.

A discectomy is performed when the intervertebral disc have herniated or torn. It involves removing the protruding disc, either a portion of it or all of it by placing pressure on the nerve root. The disc material which is putting pressure on the nerve is removed through a small incision that is made over that particular disc. This is one of the most popular types of back surgeries and which also has a high rate of success. The recovery period after this procedure does not last longer than 6 weeks. The type of procedure in which the bony fragments are removed through an endoscope is called percutaneous disc removal.

Microdiscectomy may be performed as a variation of standard discectomies in which a magnifier is used to provide the advantage of a smaller incision, thus a shorter recovery process.

Spinal fusions are performed in cases in which the patient has had the entire disc removed or when another condition has caused the vertebrae to become unstable. The procedure consists in uniting two or more vertebrae by using bone grafts and metalwork to provide more strength for the healing bone. Recovery after spinal fusion may take up to one year, depending greatly on the age of the patient, the reason why surgery has been performed and how many bony segments needed to be fused.

In cases of spinal stenosis or disc herniation, laminectomies can be performed to relieve the pressure on the nerves. During such a procedure, the surgeon enlarges the spinal canal by removing or trimming away the lamina which will provide more space for the nerves. The severity of the condition as well as the general health status of the patient are key factors in establishing the recovery time, which may be range from 8 weeks to 6 months.

Back surgery can be performed to prevent the growth of benign and malignant tumors. In the first case, surgery has the goal of relieving the pressure from the nerves which is caused by a benign growth, whereas in the latter the procedure is aimed to prevent the spread of cancer to other areas of the body. Recovery depends on the type of tumor that is being removed, the health status of the patient and the size of the tumor.

Most Advanced Cervical Discectomy Surgery in India - Best Spine Surgery Hospital India

Cervical discectomy is surgery to remove one or more discs from the neck. The disc is the pad that separates the neck vertebrae; ectomy means to take out. Usually a discectomy is combined with a fusion of the two vertebrae that are separated by the disc. In some cases, this procedure is done without a fusion. A cervical discectomy without a fusion may be suggested for younger patients between 20 and 45 years old who have symptoms due to a herniated disc.
Who is a possible candidate for Cervical Disectomy?
Neck pain is usually due to problems with a disc or facet joint. Disc pain tends to be worse when bending forwards. Facet joint problems tend to be worse with neck extension (arching backwards) and occur more often in the older population.
Disc and facet disease can cause neck pain directly, or may cause "referred" pain, meaning pain from the cervical spine which is felt elsewhere - into the neck, shoulders or shoulder blades. It is uncommon for disc or facet joint pain to radiate down beyond the elbow, but it does occur. Facet joint pain rarely radiates down past the shoulder.
Who may require Cervical Discectomy?
Those patients who suffer from persistent neck pain, one or both arm pain and weakness, instability while walking and lack of bowel and bladder control in spite of taking conservative treatment methods such as rest, medication, physical therapy are ideal candidates for this procedure.
The disc may herniate backwards to compress upon the cord or it may bulge sideways to compress upon the nerve. Cord compression may lead to numbness and weakness of both arms and in severe case may affect the bowel and bladder function. This procedure aims to remove the complete disc or its fragments in order to relieve compression of the cord or nerve and restore their function.
In Cervical Discectomy, disc can be accessed via Anterior or Posterior approach.
The Anterior approach is more popular with the surgeons as the access to disc is very easy with little muscle tissue on the way and complete disc removal is possible with no recurrence of disc herniation. This approach also provides a good exposure to all cervical vertebrae from C2 to cervicothoracic junction. The limited amount of muscle division or dissection helps to limit postoperative pain following the spine surgery. The main postoperative problem most patients face is difficulty swallowing for 2-5 days due to retraction of the esophagus.
Posterior approach is slightly complicated as too many tissues have to be traversed to reach the disc. There are chances of injury to spinal cord and nerve roots while accessing the disc. Also since complete removal of disc is not possible, re-herniation can occur. But the positive aspect of this approach is that spinal fusion is not required so natural spine movement is preserved.
What happens after surgery?
Patients are usually able to get out of bed within an hour or two after surgery. Your surgeon may have you wear a hard or soft neck collar. If not, you will be instructed to move your neck only carefully and comfortably.
Most patients leave the hospital the day after surgery and are safe to drive within a week or two. People generally get back to light work by four weeks and can do heavier work and sports within two to three months. Outpatient physical therapy is usually prescribed only for patients who have extra pain or show significant muscle weakness and deconditioning.
Rehabilitation
Patients usually don't require formal rehabilitation after routine cervical discectomy surgery. Surgeons may prescribe a short period of physical therapy when patients have lost muscle tone in the shoulder or arm, when they have problems controlling pain, or when they need guidance about returning to heavier types of work.
If you require outpatient physical therapy, you will probably only need to attend therapy      sessions for two to four weeks. You should expect full recovery to take up to three months.

Advantages of Spinal Fusion Surgery - Spinal Fusion Surgery in India

A person’s spine is subjected to a lot of forces throughout its lifetime. It may be subjected to age related wear and tear or infections like tuberculosis. As a result it undergoes degeneration which gives rise to a variety of problems like spinal osteoarthritis, disc prolapse, spondylolisthesis, osteoporotic fractures, vertebral collapse etc.

All these conditions manifest as severe pain in the back and radiating pain, numbness, tingling in the upper or lower extremities (lower leg or arm) due to compression of spinal nerves, spinal cord and spinal instability. When these symptoms become debilitating enough to preclude the patient’s day to day activity, a Spinal Fusion Surgery is advised.

What is Spinal Fusion Surgery?

Spinal fusion (spondylodesis or spondylosyndesis) is the surgical method of joining two or more verebrae. It is done most commonly in the lumbar region of the spine, but it is also used to treat cervical and thoracic problems. In this procedure, the damaged and painful vertebrae are fused into a solid stable bone. Fusion of the spine is used primarily to eliminate the pain caused by abnormal motion of the vertebrae by immobilizing the faulty vertebrae themselves, which is usually caused by degenerative conditions. However, spinal fusion is also the preferred way to treat most spinal deformities, specifically scoliosis and kyphosis. But, the results may vary according to the individual. Some may not feel any improvement in their back pain even after the surgery.
The other method is the Minimally Invasive PLIF procedure which is performed using x-ray guidance. 2.5-cm incisions are made on either side of the lower back The muscles are gradually dilated and tubular retractors inserted to allow access to the affected area of the lumbar spine. The lamina is removed to allow visualization of the nerve roots. The offending disc material is removed from the spine and replaced with a bone graft and structural support from a cage made of bone, titanium, carbon-fiber, or a polymer, followed by placement of rod and screws. The tubular retractors are removed, allowing the dilated muscles to come back together, and the incisions are closed.

This procedure typically takes about 3 to 3 ½ hours to perform. There was less blood loss, tissue trauma, operative time, and quick recovery in this procedure as compared to the traditional one.Generally the patients can go home within 3-5 days after traditional PLIF and within 1-2 days after a minimally invasive PLIF

What are the Advantages of Spinal Fusion Surgery?
In most cases, spinal fusion can eliminate the pain and other symptoms. It is important for a surgeon to explain the options, and benefits of spinal fusion surgery. There are many advantages of Spinal fusion surgery such as:

• By fusing the level, there is less concern that the patient will need surgery again at that level. If the discs and/or facets are pain generators, as is often the case, a fusion will ensure that motion at the level is stopped, potentially eliminating the low back pain.

• A second advantage of spinal fusion surgery is that it reduces post-decompressive deformity. Some patients develop post-laminectomy kyphosis, which is an abnormal alignment of the spine. This too can often lead to additional surgery. Similarly, a fusion can correct any deformity the patient had before surgery and ensure that the normal alignment of the spine is restored. This may reduce wear and tear (and need for future surgery) on the neighboring levels.

Minimally Invasive Surgery (MIS) for Spine Treatment - World's Class Spine Hospitals in India

The term "Minimally Invasive" in most cases, when describing a surgical procedure, refers to a technique using small incisions to gain access to the particular area of the body needing treatment. By avoiding a larger incision, there can be less force from the retractors on the surrounding tissues, less tissue dissection, and usually better preservation of the surrounding blood supply to the tissues in the operated area.

Ideally, these factors lead to quicker healing and a recovery with less pain, although these differences have not been proven conclusively. For an "open" procedure (non-MIS), the surgical wound is larger and more dissection is required. Retraction on the muscles, when kept in place during a long procedure, has been shown to cause damage to these muscles, with some of these effects possibly permanent.

The comparison between minimally invasive and open surgery can be made when describing a lumbar fusion. This procedure typically has the patient lying face down during the surgery, with the work done on the back side (posterior). With an open case, the incision is usually in the midline and the muscles are spread to the sides by retractors. Since the natural tendency of the muscles is to return to the midline, the force can be fairly high at the spot where the retractor is in contact with the muscles.

If the surgeon tries to minimize the length of the incision with the open technique, he/she may have to use the retractors to spread harder to see the areas where the work needs to be done. These retractor-induced forces can be reduced somewhat by, paradoxically, making a longer incision. With a minimally invasive approach, usually a tubular retractor is used.
After the appropriate level of the spine is identified with x-ray, first small, then sequentially larger tubes are placed, one over another, to dilate (expand) the opening. The force on the muscles is distributed more equally in a direction like spokes on a wheel, with less localized pressure and thus less potential for muscle damage.

While many patients will feel an attraction to the advantages of a minimally invasive approach, there still needs to be an understanding of all aspects of the surgical plan in order to make an informed decision. Patients need to know the rationale behind the proposed surgery, the chances of success and expected rehab course, and probably most importantly, the risks.
While minimally invasive spine surgery has many advantages, which are summarized below, anyone considering surgery on their spine should know that there are still potential risks, even with a small incision. An internet search of "minimally invasive spine surgery" will produce links to many websites STOCKED with shining

testimonials of how much better the patients felt after the advertised procedure, but there is often little discussion of long term follow-up, rates of recurrence of symptoms, or risk of having no relief or being worse. Minimally invasive spine surgery is still spine surgery.

To summarize, the advantages  of minimally invasive spine surgery are:

Advantages:
  • Smaller incisions, usually a few smaller scars instead of one larger scar
  • Less tissue dissection
  • Less damage to surrounding muscles
  • Potential for less blood loss, quicker healing, shorter hospital stay, and less pain
  • Quicker return to daily activities
For patients considering a minimally invasive approach, they should understand that each case is considered individually, and they should consult their doctor to see what might be appropriate treatment options after non-operative treatments have been exhausted.

Most Advanced and Latest Spinal Cord Injury Treatment in India

Your spinal cord is a bundle of nerves that runs down the middle of your back. It carries signals back and forth between your body and your brain. A spinal cord injury disrupts the signals. Spinal cord injuries usually begin with a blow that fractures or dislocates your vertebrae, the bone disks that make up your spine. Most injuries don't cut through your spinal cord. Instead, they cause damage when pieces of vertebrae tear into cord tissue or press down on the nerve parts that carry signals.
Spinal cord injuries can be complete or incomplete. With a complete spinal cord injury, the cord can't send signals below the level of the injury. As a result, you are paralyzed below the injury. With an incomplete injury, you have some movement and sensation below the injury.
A spinal cord injury is a medical emergency. Immediate treatment can reduce long-term effects. Treatments may include medicines, braces or traction to stabilize the spine, and surgery. Later treatment usually includes medicines and rehabilitation therapy. Mobility aids and assertive devices may help you to get around and do some daily tasks.
Each patient's injury is unique. Some patients require surgery to stabilize the spine, correct a gross misalignment, or to remove tissue causing cord or nerve compression. Spinal stabilization often helps to prevent further damage.  Some patients may be placed in traction and the spine allowed to heal naturally.  Every injury is unique as is the course of post injury treatment that follows.

Spinal Cord Shock

When a spinal cord injury is caused due to trauma, the body goes into a state known as spinal shock. While spinal shock begins within a few minutes of the injury, it make take several hours before the full effects occur. During spinal shock the nervous system is unable to transmit signals, some of which may return once spinal shock has subsided, the time spinal shock lasts for is approximately 4-6 weeks following the injury. In some rare cases spinal cord shock can last for several more months.  The loss of these signals will effect the persons movement, sensation and how well the body’s systems function. Often the persons loss of movement and sensation below the level of the spinal cord injury may appear complete soon after the injury. This may mask the real extent of the damage. Usually, over the first few weeks the some of body systems adjust to the effects of the injury and their function improves. Therefore, during this time and the early stage of ANY new injury it is unlikely that an accurate prediction of any recovery or permanent paralysis can be made.

Surgery

Depending on the circumstances, when surgery is required, it may be performed within 8 hours following injury. Surgery may be considered if the spinal cord is compressed and when the spine requires stabilization. The surgeon decides the procedure that will provide the greatest benefit for the patient.

Different tissue and bony structures including vertebrae misaligned from the force of injury, a herniated disc, or a haematoma can cause spinal cord compression. An unstable spine may require spinal instrumentation and fusion to build in support.

Spinal instrumentation and fusion can be used to provide permanent stability to the spinal column. These procedures correct, join, and solidify the level where a spinal element has been damaged or removed .

Instrumentation uses medically designed hardware such as rods, bars, wires and screws. Instrumentation is combined with fusion (bone grafts) to permanently join two or more vertebrae.

Healing Broken Bones Without Surgery
Surgery isn't the only way to repair broken bones in the neck and back a more conservative and less invasive approach is to allow the bones to heal naturally with the help of halo traction where needed.  This approach may mean up to two months complete bed rest.

Recovery
Once the patient is stabilized, care and treatment focuses on supportive care and rehabilitation. Family members, nurses, or specially trained carers give supportive care. This care might include helping the patient bathe, dress, change positions to prevent bedsores, and other assistance.

Rehabilitation often includes physical therapy, occupational therapy, and counseling for emotional support. Each program is designed to meet the patient's unique needs.

Laser therapy for spine Treatment in India - Best Spine Hospital in India

Laser therapy is the use of monochromatic light emission from a low intensity laser diode (250 milliwatts or less) or an array of high intensity super luminous diodes (providing total optical power in the 1000-2000 milliwatt range). Conditions treated include musculoskeletal injuries, chronic and degenerative conditions and wounds. The light source is placed in contact with the skin allowing the photon energy to penetrate tissue, where it interacts with various intracellular biomolecules resulting in the restoration of normal cell morphology and function. This also enhances the body's natural healing processes. The following chart outlines some of the specific effects of Low Intensity Laser Therapy.

The underlying science behind cold laser therapy is sound. Studies have shown that light of certain wavelengths can reduce pain and inflammation. Lasers can offer very focused beams of light which can be used to target specific areas, which means that in theoretical use, a low intensity laser calibrated to emit light of a specific wavele ngth could potentially be used very effectively to treat areas of inflammation on the body. However, like all medical devices, cold lasers need to be carefully evaluated for safety and efficacy.

Doctors generally recommend cold laser therapy as part of an overall pain management program. For people dealing with chronic pain, cold laser therapy is an option which could be used to reduce the pain, while people with persistent inflammations may also benefit. However, this therapy should not be undertaken without medical supervision, and it is generally not recommended as a replacement for other forms of medical treatment, such as the use of physical therapy to manage chronic pain.

Some practitioners of acupuncture have suggested that cold laser therapy could be used much like acupuncture and acupressure, with the beams of the laser targeting specific points on the body. These practitioners of alternative medicine rely on their training and experience to support their beliefs about the use of cold laser therapy. Stimulation of pressure points can be used to treat a number of conditions treatable with acupuncture.
A number of terms are used to refer to cold laser therapy, including low power laser therapy (LPLT), low level laser therapy (LLLT), biostimulation, soft laser, and laser acupuncture. In all cases, the technique involves exposing the skin to targeted laser beams for set periods of time and at set intervals. The light in the laser is supposed to stimulate damaged cells to promote healing and a reduction in inflammation and pain. The "cold" refers not the temperature of the laser, but to the fact that the laser is of low intensity, rather than high intensity lasers, which can burn the skin.
