Treatment For Hyperthyroidism in India

Surgery As A Treatment For Hyperthyroidism?

Surgical treatment for hyperthyroidism involves removal of almost all of the thyroid gland. The remaining part of the gland is not able to produce excessive amounts of thyroid hormone. This surgical procedure is known as a near total thyroidectomy. Surgical treatment is less common than in the past.


There are important points to know about surgery for hyperthyroidism:

•It brings rapid, permanent control of hyperthyroidism. Preparation before surgery with anti-thyroid drugs is advised; this may require several months.

•Almost all patients develop hypothyroidism following surgery.

•As with other surgical procedures, there are the usual risks of  1 to 2 percent cases relating to General anesthesia, Bleeding, Nerve damage, Wound infection, Other type of infection, Other complications

Need To Know:

If any of the following factors are present, surgery is more likely to be recommended:

•A very large thyroid.

•A nodular thyroid of any size that does not demonstrate much iodine uptake on RAIU scanning. This finding eliminates radioiodine as a choice for therapy.

•A thyroid nodule that may be cancerous.

•Moderate to severe hyperthyroidism in a child or a pregnant woman.


Preparation for surgery

It is most important to have thyroid hormone levels under control before surgery. Achieving control of the hyperthyroidism before surgery avoids the risk of a "thyroid storm." This condition, also known as a hyperthyroid crisis, is due to a failure of the body to tolerate hyperthyroidism in response to a stress, such as surgery.

•Preoperative, hormonal control is usually obtained with an anti-thyroid drug, which partially blocks release of previously manufactured thyroid hormone from the overactive thyroid gland.

•In addition, a few drops of an iodine solution are often taken daily for 10 to 15 days before surgery. These drops may decrease the size and number of blood vessels within the thyroid gland, thereby decreasing the risk for excess bleeding and other complications.


The Surgical Procedure

The operative procedure to treat hyperthyroidism is known as a near total thyroidectomy

•It is performed under general anesthesia.

•The surgeon makes an incision in the skin lines across the front of the neck and carefully exposes the thyroid gland.

•Precautions are taken to identify, isolate, and protect important structures in the area of the thyroid gland. Two are particularly important:

1.The laryngeal nerve, which is vital for the proper function of the larynx or voice box, is carefully identified and protected from trauma during this procedure.

2.The four small parathyroid glands, which are embedded in thyroid tissue and produce a hormone necessary for maintenance of blood calcium levels, are also identified and preserved.


•Most of the thyroid gland is removed.

•The surgeon usually leaves about 3 to 8 grams, which is less the 0.3 ounces of thyroid tissue.

•The procedure generally takes several hours.

•The incision usually heals well and is usually not even noticeable.


Possible Complications Of Surgery

Thyroidectomy is considered safe, with a complication rate for injury to the laryngeal nerves or parathyroid glands of less than two percent. The rate of complications is lowest when an experienced surgeon performs the procedure.

Damage to one or more of the laryngeal nerves may result in changes of voice quality and tone. Damage to the parathyroid glands may cause problems with calcium balance.

Development of hypothyroidism or to a low level of circulating thyroid hormone is not considered a complication of the procedure, since it almost always occurs after surgery.


Medical Care After Surgery
Following surgery, patients are likely to remain in the hospital for one or two days. After leaving the hospital, two types of follow-up are required. The individual needs to:

•Make a follow-up appointment with the surgeon to ensure that recovery is complete.

•Make an appointment with the endocrinologist to make sure that thyroid hormone levels decrease to normal and remain there.


If hypothyroidism develops, medical follow-up is important to make sure that an adequate thyroid hormone level is maintained with appropriate, ongoing thyroid hormone supplementation.

Low Cost Total Hip Replacement Surgery in India


Total Hip Replacement Hip Joint replacement or Total Hip Replacement is surgery to replace all or part of the hip 
joint with an artificial device to restore joint movement (prosthesis).
There are different types hip replacements. If a hemi-arthroplasty is performed, either the femoral head or the hip socket (acetabulum) will be replaced with a prosthetic device. In a total hip replacement, both the femoral head and the hip socket is replaced by the prosthetic device. 

WHAT IS ARTHRITIS AND WHY DO JOINTS WEAR OUT?

The normal joint in our body is made up of two bones which are lined by surface cartilage. The joint is surrounded by a capsule which has a thin lining of synovial cells which produce a thin layer of lubrication film. The lubrication film (synovial fluid) together with the surface cartilage (articular cartilage) acts as a shock absorber and allows the joint to move smoothly and lasts for many, many years. 
If the surface cartilage is badly damaged or if the joint surfaces are not aligned properly (example, in a shallow hip) then the cartilage will wear out much more quickly than the normal wear and tear and as a result the bone under the cartilage layer is exposed. The exposed bone starts to rub against each other and the process of osteoarthritis (wear and tear) is established. 

Osteoarthritis is therefore the result of mechanical wear and tear on a joint. The main
 feature is a loss of surface cartilage with bone rubbing on bone. This process produces pain. The body tries to relieve this pain by increasing the amount of fluid in the joint. This is why joints are sometimes swollen. The formation of bone spurs and cysts around the joint is another hallmark of osteoarthritis. 

In an arthritic hip
The cartilage lining is thinner than normal or completely absent. The degree of cartilage damage and inflammation varies with the type and stage of arthritis.
  • The capsule of the arthritic hip is swollen
  • The joint space is narrowed and irregular in outline; this can be seen in an X-ray image.
  • Bone spurs or excessive bone can also build up around the edges of the joint.
The combinations of these factors make the arthritic hip stiff and limit activities due to pain or fatigue.Diagnosis
  • The diagnosis of osteoarthritis is made on history, physical examination & X-rays
  • There is no blood test to diagnose Osteoarthritis (wear & tear arthritis)

Surgical procedure 

You are admitted to the hospital and after appropriate pre-operative tests and admission procedures you will be taken to the operating theatre. The anaesthetist will discuss with you the type of anaesthetic. Anaesthesia may be either general or regional. With a general anaesthetic you are asleep and with a regional (spinal or epidural) your legs and hips are numb allowing you to have the operation without pain. Usually the anaesthetist will either sedate you or give you a full anaesthetic if you have a spinal/epidural procedure. 

Most approaches to the hip are done with the patients lying on their side. When you are
 asleep you are positioned in a special brace that stabilises your pelvis and keeps you on your side. An incision is made along the side of your hip joint and the muscles carefully split and divided to expose the hip joint. 

The worn out joint is exposed and the femoral head is resected. This allows visualisation
 of the acetabulum (socket). The socket is then cleared of debris and a reamer is inserted to appropriately fashion the socket to accept the artificial acetabular component. 
After reaming is complete, the artificial socket is inserted. There are two types of sockets,
 (a) a cemented socket or (b) an uncemented socket. A cemented socket is cemented into the bone and an uncemented socket allows bone to grow into it. Your surgeon will advise you which is the most appropriate socket for your bone quality. 
An uncemented socket has the ability to accept a socket lining which is either polyethylene (special plastic), ceramic or metal. The liner is inserted into the socket. Ceramic and metal articulating joint surfaces have lower wear rates than plastic sockets and therefore tend to be used in younger patients. The newer plastics last a lot longer than the older ones and are appropriately used in older patients.
After preparation of the socket, the femoral bone is prepared with various instruments to accept either a cemented or an uncemented femoral component. Once the canal is prepared the femoral stem is inserted with or without cement. A trial femoral head is placed on the stem and the hip is reduced. During the trial reduction the hip is tensioned appropriately and put through a range of motion. At the same time leg lengths and stability are examined.

A person’s spine is subjected to a lot of forces throughout its lifetime. It may be subjected to age related wear and tear or infections like tuberculosis. What is Spinal Fusion Surgery?

Spinal fusion (spondylodesis or spondylosyndesis) is the surgical method of joining two or more verebrae. It is done most commonly in the lumbar region of the spine, but it is also used to treat cervical and thoracic problems. In this procedure, the damaged and painful vertebrae are fused into a solid stable bone. Fusion of the spine is used primarily to eliminate the pain caused by abnormal motion of the vertebrae by immobilizing the faulty vertebrae themselves, which is usually caused by degenerative conditions. However, spinal fusion is also the preferred way to treat most spinal deformities, specifically scoliosis and kyphosis. But, the results may vary according to the individual. Some may not feel any improvement in their back pain even after the surgery.
The other method is the Minimally Invasive PLIF procedure which is performed using x-ray guidance. 2.5-cm incisions are made on either side of the lower back The muscles are gradually dilated and tubular retractors inserted to allow access to the affected area of the lumbar spine. The lamina is removed to allow visualization of the nerve roots. The offending disc material is removed from the spine and replaced with a bone graft and structural support from a cage made of bone, titanium, carbon-fiber, or a polymer, followed by placement of rod and screws. The tubular retractors are removed, allowing the dilated muscles to come back together, and the incisions are closed.

This procedure typically takes about 3 to 3 ½ hours to perform. There was less blood loss,tissue trauma, operative time, and quick recovery in this procedure as compared to the traditional one.Generally the patients can go home within 3-5 days after traditional PLIF and within 1-2 days after a minimally invasive PLIF


What are the Advantages of Spinal Fusion Surgery? 
In most cases, spinal fusion can eliminate the pain and other symptoms. It is important for a surgeon to explain the options, and benefits of spinal fusion surgery. There are many advantages of Spinal fusion surgery such as:

• By fusing the level, there is less concern that the patient will need surgery again at that level. If the discs and/or facets are pain generators, as is often the case, a fusion will ensure that motion at the level is stopped, potentially eliminating the low back pain.

• A second advantage of spinal fusion surgery is that it reduces post-decompressive deformity. Some patients develop post-laminectomy kyphosis, which is an abnormal alignment of the spine. This too can often lead to additional surgery. Similarly, a fusion can correct any deformity the patient had before surgery and ensure that the normal alignment of the spine is restored. This may reduce wear and tear (and need for future surgery) on the neighboring levels.
